If a woman acts as a gestational carrier only, can she p on hereditary disease to the fetus?

I'm a would-be author working on a story where a woman provides no genetic material for an embryo, and is only the gestational carrier. The research I've done so far, including looking at forms for surrogates, suggests or requires that the applicant list every disease in their family that may cause an inconvenience or real trouble for an individual. Why is this, since the woman is only providing her uterus? Is any DNA transferred through the fetus's connection with her blood(the placenta)?
